OAAS/S 5: Minimal or no sedation
Responds readily to name spoken in normal tone
Normal speech
Normal facial expression
Eyes clear, no ptosis
OAAS/S 4: Mild sedation
Lethargic response to name spoken in normal tone
Mild slowing or thickening of speech
Mild relaxation of facial expression
Eyes glazed or mild ptosis
OAAS/S 3: Moderate sedation
Responds only after name is called loudly and/or repeatedly
Slurring or prominent slowing of speech
Marked relaxation of jaw
Eyes glazed or marked ptosis
OAAS/S 2: Deep sedation
Responds only after mild prodding or shaking
Few recognizable words
OAAS/S 1: General Anesthesia
Does not respond to mild prodding or shaking
